Key Responsibilities:
• Collaborate within a person-centered care team
• Conduct resident assessments and maintain accurate care plans
• Implement and evaluate care plans,

• Respond to medical emergencies and significant health changes
• Educate residents and families regarding health conditions

Requirements:
• Current registration as a Licensed Practical Nurse
• Experience in senior or long-term care preferred
• CPR and First Aid certification
• Proficiency with electronic documentation
• Solid clinical and communication skills
