- Completion of recognized Nurse Practitioner program
- Current extended class certification with College of Nurses of Ontario
- Experience with chronic disease management and geriatric care
- Current vulnerable sector police check

Job Responsibilities:

- Responsibility over medical care delivery to LTC residents
- Management of stable complex chronic co-morbidities in collaboration with MDs
- Collaboration with an interdisciplinary team in the course of care delivery
- Provide high quality customer service
- Provide clinical / medical education to clients and staff
- Participate in care conferencing




- Role is 70% clinical and 30% quality improvement, education, and leadership
