We are seeking a dedicated and compassionate Nurse Practitioner (RN/RPN) to join our agile healthcare team at Behaviorprise. In this vital role, you will provide comprehensive patient care, collaborate with multidisciplinary teams, and deliver high-quality medical services across various settings. Your expertise will help improve patient outcomes, support families, and enhance the overall quality of care. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient's from children to adults and senior individuals that are diagnosed with Autisim Spectrum disorder (ASD) and/or a developmental disability. We are approved child care service provider under the Ontario Autism program.
Responsibilities
- Conduct thorough assessments of patients’ health status, including history-taking and physical examinations.
- Develop and implement personalized care plans tailored to individual patient needs.
- Administer medications, including injections and IV therapies, following established protocols.
- Manage acute and chronic conditions with a focus on pain management and symptom control.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate care.
- Provide education to patients and families about health conditions, medication usage, and lifestyle modifications.
- Document all patient interactions accurately using electronic health record systems in a confidential manner .
- Perform specialized procedures and provide well curated care guide for the patient and patient's family using the principles of applied behaviour analysis (ABA) and general support strategies
- Assist in case management activities to ensure seamless continuity of care across settings such as nursing homes, assisted living facilities, hospitals, or home care environments.
- Support laboratory testing procedures and interpret results to inform treatment decisions.
Skills
- Proven experience working with children (pediatrics) and senior populations in various healthcare settings.
- Strong background in nursing home, hospital, or assisted living environments with expertise in acute care and dementia care.
- Excellent knowledge of medical terminology, medication administration protocols, and laboratory procedures.
- Proficiency in electronic health record systems such as Epic or Cerner for documentation and order entry.
- Skilled in case management practices to coordinate comprehensive patient care plans effectively.
- Experience in pain management techniques and home care services delivery.
- Ability to perform phlebotomy confidently and manage laboratory sample collection accurately.
- Familiarity with medical office operations and medical terminology for efficient workflow management.
- Compassionate approach to patient care with excellent communication skills for engaging diverse populations including pediatrics and elderly patients.

Pay: $30.00-$65.00 per hour
