Position: Care Coordinator RN, PT, OT or SW #075-26 CARE AND BE CAREDFOR – THIS IS YOUR HOME Are you an experienced registered nurse,physiotherapist, occupational therapist, or social worker seeking a rewardingcareer that cares for others, in a professional practice that cares for you?As a Care Coordinator (Status:Full-Time,Startdate:Immediately;Location:Timmins) you will assess and determine patient careneeds and eligibility, provide access and referrals to community services, and engagewith patients, caregivers and other health care practitioners. Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals. In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans Coordinate and monitor care plan delivery Establish a helping relationship with patients and their families Membership, in good standing, with the applicable regulatory body:
Collegeof Physiotherapists of Ontario Collegeof Occupational Therapists of Ontario OntarioCollege of Social Workers and Social Service Workers Recentexperience in community health or a related field Knowledgeof the health care delivery system and community resources Advanceoral and written proficiency in English and French is essential Wehave a mandatory COVID-19 vaccination policy. As a condition ofemployment, all employees are required to submit proof of COVID-19vaccination status prior to start date.
