- Collaborate on individualized service plans with clients
- Assist clients in meeting CTO obligations and accessing services

- Foster respectful relationships with clients and support systems
- Deliver advocacy to support client needs in the community
Requirements:
- Degree or diploma in social services or related field
- Three years' experience in community mental health
- Strong knowledge of trauma-informed care practices
- Ability to analyze and make decisions effectively
- Valid Ontario Driver's License required
