Key Responsibilities
- Direct integrated palliative care team operations • Collaborate with care teams to establish comprehensive care plans • Monitor and oversee indirect care coordination practices • Provide high-level support for symptom management and care advocacy • Build strong stakeholder relationships to improve care quality Requirements:
