The Clinical Dietitian is responsible for the assessment, education, and treatment of patients at the Sioux Lookout Meno Ya Win Health Centre who require nutrition support. Bachelor’s Degree in Applied Nutrition or equivalent. Completion of Dietetic Internship.
Registration with the College of Dietitians of Ontario, or eligibility to obtain. Work within the Registered Dietitian scope of practice as determined by the College of Dietitians of Ontario. Screen, prioritize and process client referrals using information obtained through the physician, laboratory reports and health records.
Complete nutrition and health assessments to develop nutrition care plans with clients by integrating information about the client’s health status and available community resources. Develop, implement and evaluate in-hospital diabetes management and facilitate ongoing outpatient follow up, Be able to identify and prioritize complex health and social issues, Evaluate nutritional status of individuals and aid in the prevention and/or treatment of inadequate nutrition. Provides direction to the Manager Food & Nutrition Services and the multidisciplinary team to meet the nutritional needs of residents, including regular menu review and approval.
Monitors and evaluates the nutritional status of each resident at William George Extended Care, including their current nutritional risk, prioritizing as required; documents these reviews as per MOHLTC standards and regulations. Communicate information clearly and concisely to clients and healthcare providers regularly. Acts as a nutritional resource person for clients, their families, other health professionals and support agencies.
Maintains a research-based practice and participates in research projects, as appropriate. Must adhere to organizational policies and procedures concerning cleaning, hand hygiene and the use of Personal Protective Equipment in support of Infection Control measures and attend training when necessary.
