Job Summary

In keeping with the Mission, Vision, and Values of Park Place, the LPN/RPN collaborates with the interdisciplinary team to use the nursing process and apply nursing knowledge, skills, and clinical judgment in performing assessments, planning, implementing, and providing quality and person‑centred nursing care to residents. The LPN/RPN operates in accordance with the competency guidelines, full scope of practice, provincial governing body practice standards, Code of Ethics, and Park Place policies and procedures, committing to Park Place’s culture of safety.

Duties and Responsibilities

- Demonstrate nursing knowledge and the ability to utilize the nursing process to identify residents’ health care needs and achieve mutually agreed health outcomes.
- Apply critical thinking and problem‑solving skills in all aspects of care.
- Prioritize and manage resident care according to identified needs.
- Provide support and establish therapeutic relationships with residents and their families.
- Use computer skills for documenting resident care, sharing information with the health care team while respecting confidentiality, and validating evidence‑based practice.
- Advocate for residents’ rights: obtain informed consent, promote self‑determination, and adhere to Park Place’s Residents Bill of Rights.
- Apply the Code of Ethics for LPN/RPN to guide decision making and practice.

Clinical Practice

Assessment and Care Planning
- Utilize standardized assessment tools (e.g., MDS‑RAI, User Defined Assessments).
- Perform move‑in, move‑out, and resident transfer processes efficiently.
- Conduct comprehensive health and psychosocial assessments, process orders, document and report, provide health teachings, perform medication reconciliation, care planning, advance care planning.
- Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans.
- Document all assessments accurately in the electronic health record.

Medication Management




- Understand pharmacology and medication administration principles.
- Perform resident assessment prior to medication administration, observe the rights and checks.
- Administer medications per Park Place policies and provincial guidelines and monitor effectiveness.
- Initiate, discontinue, and manage hypodermoclysis infusions as ordered.
- Maintain accurate drug counts for controlled medications.
- Report adverse reactions and medication errors per legislation and policies.

Procedures and Interventions
- Perform head‑to‑toe assessments, record and interpret vital signs, assist with activities of daily living, manage elimination, nutrition, and hydration, provide basic foot care, basic wound care, palliative care, and post‑mortem care as per provincial competencies.

Communication and Technology

- Communicate fluently in English, both verbally and in writing.
- Apply therapeutic communication techniques.
- Document and update all relevant care information to ensure resident safety and comfort.
- Share information respecting confidentiality.
- Perform clerical duties such as answering phone, making appointments, ordering supplies, initiating diagnostic requisitions.
- Apply nursing informatics: adapt to electronic processes, complete UDA, clear alerts, enter orders, report incidents, update electronic health records.
- Understand risks of inappropriate social media use.

Health and Safety Responsibilities

- Read, understand, and comply with Park Place and provincial OHS policies.
- Participate in the Health and Safety Program: use safe work procedures, promote awareness, suggest safety improvements, read OHS minutes.
- Take precautions to protect resident and worker safety, adhering to emergency procedures and infection control guidelines.
- Report near‑misses, injuries, equipment damage, and unsafe conditions promptly to supervisor or nursing manager.




- Correct or report unsafe conditions and equipment malfunctions.

Professionalism and Leadership

- Demonstrate professional presence and behavior.
- Establish and maintain professional relationships with the interdisciplinary team.
- Maintain appropriate professional boundaries with residents and families.
- Use conflict resolution skills to foster collaboration.
- Supervise staff: schedule, coordinate assignments, evaluate performance, determine training needs, and assign care appropriately.
- Organize workload, demonstrate time‑management skills.
- Assist orientation and information sharing with current staff and students.
- Participate in quality improvement and program evaluation activities.
- Seek qualified growth opportunities.
- Perform other related duties as assigned.

Qualifications

- Education: Graduation from a recognized Licensed Practice Nurse (LPN) or Registered Practical Nurse (RPN) program.
- Experience: At least one year of recent related experience.
- Registration/Certification: Maintain and provide proof of active registration with the provincial College (e.g., Alberta College of Nursing, British Columbia College of Nursing Professionals, College of Licensed Practical Nurses Association, Registered Practical Nurses Association of Ontario). First Aid certification required.

- Computer, iPad, iPod, telephone, facsimile, photocopier.
- Vital signs machine.
- Mechanical lift.
- Medication/Wound cart.

Physical Demands

- Requires lifting, walking, reaching, bending, pushing, standing, and sitting.
- Spending 2–6 hours standing or walking per day.
- Lifts up to 50 lbs.; frequent lifting of 20 lbs.
- Must manage stressful situations with dexterity and professionalism.

Health and Safety Risks – Environment

- Physical: Strain on shoulder, neck, back; cuts, scrapes, injections.
- Environmental: Drafts, dust exposure, working alone.
- Chemical: Equipment cleaners.
- Biological: Exposure to communicable diseases (e.g., colds, influenza, GI).
