Assist in delivering exceptional care as a Long Term Care Registered Nurse. Focus on nursing assessments, treatment implementation, and patient education while advocating for individual needs. This role is ideal for RNs who enjoy working in a collaborative environment.
You will evaluate treatment effectiveness, document patient responses, and facilitate appropriate care planning and education. The RN will also serve as a patient liaison, ensuring all needs are met according to established healthcare standards. Key Responsibilities:
- Conduct thorough nursing assessments for patients
- Implement and evaluate treatment and care plans
- Document responses and treatment progress effectively
- Act as a liaison between patients and healthcare teams
- Provide education and facilitate discharge planning
Requirements:
- Must be a graduate of a Registered Nurse program
- Active registration with the College of Registered Nurses needed
- Basic Life Support for Healthcare Providers (Level C)
- Required courses if not practiced in the last 5 years
- Robust advocacy and coordination skills
