About the Role

Reporting to the Director of Clinical Mental Health and Wellness, the Manager of Assertive Community Treatment Team (ACTT) provides operational and clinical leadership across two multidisciplinary teams. This role ensures high-fidelity service delivery in alignment with Ontario Association for ACT & FACT (OAAF) standards, organizational priorities, legislative requirements, and evidence-informed recovery practices for adults living with severe and persistent mental illness and concurrent disorders.

Key Responsibilities

Clinical Leadership & Governance: Provide clinical oversight, risk management, crisis response, and triage for complex client situations; champion recovery-oriented, trauma-informed, anti-racist, and person-centered care.

Program Operations: Lead daily operations, interdisciplinary clinical meetings, workload allocation, and scheduling for 7-day service coverage and 24/7 crisis response availability across two interdisciplinary teams (Psychiatry, Nursing, Social Work, OT, Addiction/Vocational Specialists, Peer Support).

Quality Improvement & Fidelity: Coordinate internal and external OAAF fidelity reviews, monitor program KPIs (hospitalization rates, service intensity, OCAN completion, client outcomes), and lead continuous quality improvement (CQI) initiatives.

Staff Leadership & Development: Recruit, mentor, evaluate, and supervise clinical staff; conduct in-field (in vivo) coaching and side-by-side community supervision to promote a safe, inclusive, and collaborative team culture.

Resource & Financial Management: Develop, monitor, and manage program budgets, optimize resource allocation, and participate in strategic business planning and funding proposals.

System & Community Partnership: Build strategic relationships with Ontario Health Teams, hospitals, primary care networks,



housing providers, emergency services, and justice partners to facilitate seamless transitions across the continuum of care.

Qualifications

Education: Master’s Degree in Social Work (MSW), Nursing (MScN/MN), Occupational Therapy (MOT), Psychology, or a related regulated health discipline.

Licensing: Active registration in good standing with the applicable regulatory college (e.g., OCSWSSW, CNO, COTO, CPO, CRPO).

Experience: Minimum 5 years of progressive clinical leadership or supervisory experience within community mental health.

Technical Knowledge: Deep expertise in Assertive Community Treatment (ACT) philosophy, psychosocial rehabilitation, and OAAF Program Standards.

Legislative Expertise: Comprehensive knowledge of Ontario health legislation, including the Mental Health Act, Health Care Consent Act, PHIPA, and Substitute Decisions Act.

Driver's License: Valid Ontario Class G Driver’s License with access to a reliable vehicle and appropriate insurance.

Pay: $73,000.00-$105,000.00 per year
