In this valuable role, the Clinical Unregulated Care Provider Supervisor is accountable for the supervision of PSWs and HSWs who are caring and making a difference for clients within the community. The Clinical UCP Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care. Respond to and advise/resolve concerns from community staff, patients/families, and other team members. Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.

Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals. Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
