Key Responsibilities





- Serve as the key worker for a defined patient caseload • Implement holistic assessments and care strategies • Facilitate physical health monitoring and treatments • Address patient vulnerabilities and mental health needs • Work in partnership with multidisciplinary teams Requirements:
