WELL Health Diagnostic Centres is the largest operator of independent diagnostic health facilities in Ontario. We provide cardiology, radiology, respirology, and sleep diagnostic services at 60+ accredited locations across the province.
Responsibilities
- Conduct comprehensive health assessments, including medical histories and physical examinations in collaboration with cardiologists and Internal Medicine specialists.
- Provide patient education and counseling regarding disease prevention, treatment options, and healthy lifestyle choices.
- Monitor patient progress and adjust treatment plans as required.
- Collaborate with physicians, specialists, allied health professionals, and community resources to ensure continuity of care.
- Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system.
- Participate in quality improvement initiatives and support best practices in patient care.
- Adhere to all regulatory, professional, and organizational standards.
- Perform consultations and stress test supervision as assigned.
- Perform all medical tasks delegated by the attending physician.
- Ensure a safe, clean working environment and call for assistance when required.
- Document patient care services by charting in patient records and recording vitals, history,
and prescriptions.
- Perform any other tasks assigned by the supervisor.
Qualifications and Experience
- 0-2 years of related experience.
- Registered Nurse Practitioner certification is required.
- Member in good standing with the College of Nurses of Ontario.
- Demonstrated knowledge and understanding of general medicine.
- Ability to collaborate and communicate effectively with health professionals and patients.
- Ability to function effectively in a clinical setting.
- Familiarity with electronic medical records (EMR).
- Competence in customer service, communication, teamwork, initiative, accountability, dependability, flexibility, and adaptability.
- Computer literacy in MS Office and ability to research on the Internet.
- Work effectively within a diverse community and show empathy toward patients and families.
- Work independently or as part of a multidisciplinary healthcare team.
- Commitment to continuing education and training.
- Effective decision‑making and problem‑solving skills.
