Monitor the provision of Elder care by the nursing team on an assigned neighbourhood, acting as the team leader. Assess the Elder’s needs in accordance with theInterRAItool and provide a plan of care and interventions based on an evaluation of outcomes.

Elder response to medication and treatments to physician/nurse practitioner Chart care provided, behaviours and incidents CompleteInterRAI assessments, CAPs and care plans according to schedule Provide guidance and direction to Personal Support Workers, and other staff working on the assigned unit Elder Care:



Assist as necessary with Elder care and transferring Monitor levels of liquid oxygen in portable tanks and fill as necessary Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed Incumbents must possess the physical ability to meet the duties and responsibilities of the and/or the requirements identified within the position’s Physical Demands Analysis These describe the general nature and level of work being performed by incumbents in this classification.
