To apply the nursing process utilizing a patient- and family-centered approach in providing holistic, quality and safe care to patients within the scope of practice identified by the College of Nurses of Ontario, evidence-based nursing practices and the standards set by Lakeridge Health. To actively participate as a member of both the Health Care Team and the Nursing Team. To ensure accurate and timely documentation of nursing records.

To provide education and training to patients, their essential care partners, community agency staff, and other stakeholders. To perform peritoneal dialysis both for acute admitted patients as well as in the home environment,



including managing all aspects of care for peritoneal dialysis patients, and providing ongoing advice, counselling, and education related to their condition. Must be able to travel to complete patient home visits and between Lakeridge Health sites as required.

Current registration with the College of Nurses of Ontario (in good standing)

- Demonstrated commitment to maintain Reflective Practice Standard as outlined by the College of Nurses of Ontario
- Home peritoneal dialysis experience required Demonstrated recent (within 2 years) clinical experience in Peritoneal dialysis Nephrology certificate or evidence of progression toward completion required
- Understanding of the principles of adult education and demonstrated ability to apply those principles Recent experience with the equipment related to home therapies including but not limited to peritoneal dialysis equipment Previous experience in the area of patient education an asset Physically capable of performing the duties of the position in order to lift, turn, and transfer patients manually and with mechanical aids
- Demonstrated excellent organizational,
