Patients may be: Working toward increased independence through rehabilitation Receiving palliative and comfort-focused care Our Nurse Practitioner-led model promotes a collaborative approach that allows team members to build meaningful relationships with patients and contribute to their ongoing care journey.

and palliative care Build meaningful connections with patients through ongoing care relationships Opportunities to expand your clinical skills and healthcare experience Make a lasting difference in the lives of patients and families What You'll Do As a Patient Care Assistant, you will: Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility Support patient comfort, safety, and independence Assist with nutrition, hydration, and meal support Observe and communicate changes in patient condition to nursing staff Document care activities accurately and according to policy Help maintain a safe, organized, and welcoming patient care environment Collaborate with nurses and the interdisciplinary team to deliver exceptional patient-centered care What You Bring We are looking for individuals who are: Compassionate, dependable, and patient-focused Strong communicators who work well within a team environment Comfortable working independently while following direction Dedicated to providing respectful, high-quality care Requirements: Graduate of a Personal Support Worker program (or equivalent) within the past two years; or one year of full-time equivalent experience within the past five years Secondary school diploma or equivalent Minimum 150 hours of PSW (or equivalent)
