• Clinical Support Provide skilled nursing care within RN scope of practice Perform tracheal suctioning as needed Administer medications (oral, injectable, topical) Perform wound care, dressing changes, and skin assessments Monitor vital signs and assess changes in client condition Support chronic disease management and symptom control Communicate health concerns promptly to the Case Manager or healthcare team Daily Living &

• Safety Support Assist with personal care when required (bathing, grooming, toileting) Support safe mobility, positioning, and transfers Reinforce safety strategies and fall‑prevention measures Documentation &

• Communication Complete accurate nursing notes for each visit Document observations, significant changes, and interventions Collaborate effectively with families and the interdisciplinary team Companionship &
