The Salvation

As a member of the health care team, assists in the assessing, planning and implementation and evaluation of resident care. Assumes significant responsibility in ensuring continuity of care through recording and reporting.

Accountabilities: Reporting to the Registered Nurse, the RPN is responsible for the provision of individualized physical and emotional care and support for residents. Fosters a team spirit. Participates in shift report with oncoming and outgoing RPNs/PSWs.

Demonstrates ability to adapt to changing priorities and serves as a role model in current nursing trends and practices. Respond immediately when portable phone is ringing. Participates in quality improvement, risk management, infection control, and health and safety activities within the Nursing Department.

Participates in the planning and implementation of the residents’ individualized plans of care, when care needs are well-defined with predictable outcomes. Acts as a resource with the RPN when resident care needs become more complex, with unpredictable outcomes. Provides and/or assists with the personal care of the residents, as required.

Evaluates care levels and reassigns staff accordingly to assure provision of safe nursing practices. Participates in care conferences. Reviews physicians’ orders; Delegates some aspects of care to PSW’s but retains overall accountability for care.

Documents Clinical

Records in an accurate and concise manner in accordance with CNO Standards.



Assesses residents and reports promptly to the RN and POA/family member of any changes in a resident’s condition. Administers medication, controls narcotics in accordance with established procedures.

Counts narcotics with another Registered Nursing staff at the beginning/end of each shift. Seeks advice when required from the Registered Nurse prior to administering prn medications per policy. Participates in Nursing Continuous Quality Improvement audits.

Acts as a second relay in Resident call bell alarms. Carries out nursing procedures in accordance with the College of Nurses of Ontario Standards and evidence-based practice. Adheres to Ministry of Health and Long Term Care Standards.

Adheres to Grace Manor’s policies & procedures. Demonstrates and shares nursing best practices in daily routines. Provides assistance in taking residents to planned social and recreational activities. completing assessments, coding, RAPS, care planning workload).

Completes clinical documentation to support RAI-MDS assessments, coding, and RAPS. Supervises PSW’s to ensure flow sheets are completed accurately to support RAI-MDS assessments, coding, and RAPS.

Quality

Improvement/Risk Management Monitors efficient use of equipment and supplies on unit; reports equipment malfunction and/or supply needs to the Director of Environmental Services/Director of Resident Care.

Attends Staff

Development and In-service Education programs to ensure personal and professional growth. May be designated as an Area Warden in Fire/Safety emergencies. Maintaining a protected and secure environment for residents, co-workers, volunteers, and visitors by following facility policies and procedures; Assists with the orientation of new employees and actively participates in the training of PSWs and new RPNs, as required.

Provides input to the RN regarding performance of PSWs.



May be required to supervise building. MANAGERIAL/Technical Leadership RESPONSIBILITY Reports directly to Charge nurse (RN) and ADOC/ Director of Care Personal Support Workers.

FINANCIAL AND MATERIALS MANAGEMENT Long term care facility, Unit using med cart etc Travel is not expected for this position May encounter disgruntled staff or resident/family representatives Much of this position is on foot with minimal sitting. Current registration as a Registered Nurse with the College of Nurses of Ontario. Proof of successful completion of the Medication Administration Course for RPN’s.

Post RN education in dementia and Supervisory skills is an asset.

Experience in geriatric nursing. Certificate in Geriatric Health Assessment preferred. Ability to safely and efficiently dispense resident medical and treatments. Excellent verbal and written communication skills in English.

Knowledge of computer operations and software applications. Must possess the personal qualities and attitudes that respect and maintain the spirit, dignity and individuality of the residents. Sound clinical judgment, tact, and personal ability to work cooperatively and effectively with residents, family members, physicians, volunteers and co-workers as a member of an Interdisciplinary Care Team.

Ability to develop and maintain rapport with elderly residents. Works closely with other disciplines to ensure that residents have their physical, psychological, social and spiritual needs met.
