Position Vacancy: Home Unit - Hazelglen Outreach Clinic

Position: Full Time Registered Practical Nurse (UNIFOR Service)

Posting Period: September 4-9

Hours of Work: Monday to Friday, Days (Some Evenings)

Location: WRHN @ Midtown (formerly Grand River Hospital, KW Campus)

Position Summary :

Offering person-centered care and guided by recovery principles, the RPN provides psychiatric and mental health nursing care for individuals 18 years or older who require individual support, family education, crisis management, mental health education, and medication management. The RPN will work as a member of an intra-professional clinical team, including social work and psychiatry. The RPN will work primarily Monday-Friday, with flexibility required for occasional evenings to meet patient and family needs.

The Hazelglen Outreach Clinic is a voluntary service for patients who present in crisis to the ED who require short term community follow-up and support. Hazelglen provides intensive community case management and timely mental health treatment and support for individuals living in community settings who are experiencing an acute mental health crisis. The RRN will help assess and monitor patients’ responsiveness to medication and will be provide education and support to patients and families regarding treatment and self-care.





This position provides psychiatric treatment monitoring and support, based on the recovery model.

Position Requirements:

· Registered Practical Nurse with certificate of competence with College of Nurses of Ontario

· Post diploma in Psychiatric Nursing preferred

· Three years experience in psychiatric nursing required

· Case Management experience preferred, with ability to develop robust community agencies relationships

· Knowledge of community resources preferred

·

· Experience in working collaboratively in multidisciplinary setting

· Heart Saver and de-escalation certificates (current)

· Recognized medication course

· Knowledge of mental illness, suicidal behaviors and current mental health practices and treatment modalities

· Excellent communication, organizational and interpersonal skills

· Using preset standards and protocols within their RPN scope of practice, provides assessment, planning, intervention and evaluation of individuals with psychiatric/mental health disorders

· Working knowledge of the Mental Health Act and Community Treatment Order Legislation, Privacy Act, Health Care Consent Act, and other relevant legislation.

· Demonstrated ability to provide in depth mental health assessments, risk assessments, mental status examinations, and development of crisis, and discharge plans

· Excellent patient teaching skills

· Application of Family Systems Nursing in the provision of direct patient family care

· Valid Ontario Driver’s and access to a vehicle

· Good attendance record
