Wesley seeks a compassionate Health and Wellness Navigator to provide critical support for individuals in emergency housing. This full-time prospect focuses on mental health assistance and harm reduction strategies. - Conduct thorough assessments and service planning
- Assist with crisis resolution and participant safety plans
- Foster relationships through regular participant engagement
- Oversee harm reduction supplies and training programs
- Collaborate with community partners on treatment options Requirements:
- At least three years in a related role
- Degree in Human Services, Social Work, or similar
- Preferred registration with OCSWSSW
- Certification in Mental Health First Aid is advantageous
