Summary of DutiesTo provide a full range of social work services to patients and their families at the Holland Orthopaedic and Arthritic Centre. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.Qualifications/SkillsMasters Degree in Social WorkMember of the Ontario College of Social Workers and Social Service WorkersMinimum two (2) years of experience working in a Hospital settingExceptional communication skills requiredFamiliarity with issues of capacity and consentIn-depth understanding of the Sunnybrook Discharge Policy and its applicationStrong negotiation and conflict resolution skillsAbility to be a team player as well as self-directedAbility to manage multiple tasks and set priorities in a rapid-paced environmentCommitment to student education
