We value health equity and inclusion and respect in our work and in the delivery of our services.The Program Manager, Chronic Disease is responsible for the coordination and oversight of DECNET, Tele-Ophthalmology, Community Dietitians, and Choose Health programs. In this role, the Program Manager is further responsible for the supervision and support of a diverse team of staff, volunteers and students, including hiring, performance management, disciplinary action and decisions regarding termination of employment. Leading these programs, this position plays a lead role in responding to changing needs and trends through the planning, development, implementation and evaluation of programming to meet the needs of communities in east Toronto.Working onsite is required.ResponsibilitiesTalent Management and Supervision of StaffDirect and manage the work of interdisciplinary teams (including peer workers, students and volunteers)With support from the Director, conduct all required recruitment activities to ensure the programs are appropriately staffedProvide supervision and ongoing guidance, feedback, recognition and coaching of the program staffSupport staff development opportunities and engage staff appropriately in decision makingConduct probationary and annual performance reviewsSupport the development and maintenance of a strong teamSupport staff wellness activitiesWork with program staff to resolve problems as they ariseMonitor attendance and address concernsEnsure that staff's statistical encounters, charting,



monthly time records and all administrative tasks are completed in a timely mannerAddress performance issues as they occur and make effective recommendations with respect to disciplinary steps up to and including termination, in consultation with the DirectorEnsure all appropriate employee information is tracked and documented according to agency policiesCoordinate team meetingsProgram ManagementEnsures program services meet national/best practice guidelines for diabetes care, coordinates chart audits / surveys / evaluations, etc.Coordinate daily/weekly/monthly/annual priorities, work plans and workflow for all identified programs, in conjunction with the DirectorEnsure supportive, strengths-based, trauma-informed, anti-racist/anti-oppressive social justice and harm reduction approaches are utilized in all aspects of program planning and direct serviceEnsure the successful coordination of client-centered servicesDevelop targets, timelines and processes to successfully manage tasks and workflowAddress day to day program and client related issues and ensure these are resolved in a clear, qualified, timely mannerDevelop and coordinate inter-professional team approaches to meet the complex short and longer-term needs of clients/participants and to address the social determinants of health including utilizing crisis intervention, counselling, case management and referral as needed.Monitor, support, train and evaluate the ongoing development and implementation of the Diabetes, Tele-Ophthalmology, Community Dietitians and Choose Health related servicesIdentify and respond to increased risks, trends and gaps in programs, by offering services and support that respond to community feedback and meet diverse needsGuide and support program staff in responding to and providing servicesDevelop and maintain collaborative team programming and partnerships within Community Health and Chronic Disease and other inter-disciplinary teams and programs at SRCHC, as well as with community partners and other service agenciesDevelop program workplans in consultation with team, while aligning with organizational strategic planLead quality improvement initiativesSupport accreditation and standards recognition activitiesLead program data collection, program evaluation, chart audits, client surveys etc.Maintain records, reports and other statistical information, and provide accurate and timely reports, stats,



stories of impact to ensure program accountabilityMake recommendations related to supplies and other purchases and monitors budgets and expenditures in conjunction with the DirectorEnsure all funding reports are submitted as per accountability agreements and prepares funding proposals for review by DirectorDevelop, and coordinate effective print and electronic promotional information and materials, and strategies to outreach to individuals who may benefit from the Diabetes, Tele-Ophthalmology and Choose Health programs; organize and give presentations for diverse audiencesDevelop outreach strategies with team to meet client and community needsAll other duties as requiredQualifications:Graduate degree or significant experience in health care, social services, adult education or related field2-5 years progressive experience coordinating programs, managing and supervising staffRegistered Regulated Health Care Professional preferredCurrent college membership as regulated under the Regulated Health Professions Act, 1991 maintained as applicableDemonstrated ability to supervise and support staff to ensure client-centred, effective and efficient work processesFamiliarity with, and experience in, self-management and chronic disease programmingExperience working with vulnerable and diverse communitiesDemonstrated ability in project coordination and management and program developmentExperience with outreach initiatives and building strong partnershipsDemonstrated experience in integrating initiatives across multiple sectors; with a range of different organizations and individualsSolid interpersonal, supervisory and critical thinking skillsStrong organizational skillsExcellent verbal and written communication skillsProficiency in the use of computer technology and various software applicationsAbility to work independently and as a member of a multi-disciplinary teamPolice clearance documentation will be required for employmentProof of double COVID-19 vaccination status will be required for employmentThis posting is for an existing vacancy.The deadline for applicants is September 3, 2026 at 11:59 p.M.This is a non-bargaining unit position.SRCHC welcomes applications from people with disabilities.
