Assess the Elder's needs in accordance with theInterRAItool and provide a plan of care and interventions based on an evaluation of outcomes.Duties and ResponsibilitiesAdministration of Medications and Treatments:Administer and document medications and treatments for all Elders, and assess effectivenessReport Elder response to medication and treatments to physician/nurse practitionerTranscribe and process all orders receivedCommunication and Documentation:Chart care provided, behaviours and incidentsRespond to Elder and family inquiriesEducate Elders, families, and staffCompleteInterRAI assessments,
