At Turning Leaf, you will deliver person-centered support to participants in various living arrangements. Collaborate closely with Clinical Case Managers to create and execute programs promoting learning and healthy life choices. Key Responsibilities: • Manage participant documentation and care plans • Collaborate on incident management and reporting • Engage participants in personal goal-setting efforts • Report health and safety concerns proactively • Assist with additional duties as directed
Requirements: • Must be at least 18 years old • Two years experience in direct support required • Valid certification in First Aid / CPR • Background checks must be provided • Experience in human services beneficial
