Enhance community mental health with Fraser Health as an LPN in Mental Health Outreach. In this role, you'll provide vital nursing services to clients in their homes and community settings.Join an interdisciplinary team focused on delivering high-quality mental health care. Your responsibilities will include assessing clients, administering medications, and adjusting care plans in response to changing client needs. You will play a key role in facilitating effective communication, ensuring patient safety,



and collaborating with other health professionals.Key Responsibilities:Administer prescribed medications safely and effectivelyMonitor and evaluate clients' care plans regularlyProvide support during interdisciplinary meetingsDocument care accurately for patient safetyParticipate in quality improvement initiativesRequirements:Graduation from a recognized Practical Nursing programCurrent registration with the BCCNM as a Licensed Practical NurseFirst Aid, CPR, and Food Protected certification requiredValid BC Driver's License and vehicle accessCompletion of MHSU Competency Education or eligibilityBring your nursing skills to the forefront of mental health outreach at Fraser Health, where compassionate care is at the heart of what we do.
