POSITION SUMMARYThe Personal Support Workers – Assisted Living Programs (PSW‐HALP) enhance the quality of life of clients by providing quality and compassionate care in the form of personal care assistance, medication reminders, physical and cognitive support, and home‐keeping assistance. PSW‐HALP assists in enabling individuals to continue living independently in a protected and healthy manner in their own home/apartment units, within the various Halton Assisted Living program locations and the surrounding community.The PSW‐HALP works cooperatively with the Manager‐Assisted Living Programs (M‐HALP) and the Supervisor‐Assisted Living Programs (S‐HALP) in providing client‐centered care and in the effective execution of daily tasks. PSW‐HALP adheres to daily care schedules and overall care plans. HALP clients are typically individuals who have been discharged from hospital but are unable to return to their pre‐hospital living arrangements. These clients are in the process of having long‐term care needs assessed.They are generally medically stable, can be of any adult age group or elderly age group, and may require mobility assistance. Client stays are usually short‐term in length, and they may require a care plan.Core Duties And ResponsibilitiesAssisting clients with daily personal care activities, in keeping with their individual care planBathingDressing/undressingPersonal hair careShaving/groomingMouth carePreventative skin careOther personal hygiene tasksAssisting clients with daily housekeeping activities, in keeping with their individual care planLight apartment housekeeping (sweeping, tidying, dishes, etc.)Laundry servicesLight meal preparation / safe food handlingAssisting clients in daily activities that maintain adequate levels of physical health and safety,



in keeping with their individual care planPerforming safety wellness checksMedication remindersLiving area visual scansEngaging clients in positive/social conversationsEncouraging clients to direct and/or participate in their own careAssisting clients with general mobility needs (walking, movement, positioning, etc.) in and around their home and the program locationEncouraging participation in HALS social and/or recreational activitiesAssisting with basic range of motion exercises where required by the care planDemonstrating sensitivity and respect to clients who may be experiencing physical health challengesProviding emotional and social support to both clients and their family membersDemonstrating a positive attitude and approach towards clients as unique individualsPerforming daily work in keeping with the established Assisted Living procedures, processes, and expectationsAnswers call bells in a safe and timely mannerObserves and records client information/comments/issues/behaviours/changes on the 24‐Hour Shift ReportFollows daily client care schedules as determined by the Manager or SupervisorMaintains a clean and organized work environmentReports critical client care information/issues to Manager or Supervisor right awayUnderstands and follows individual client care plans, seeks clarification from the Manager or Supervisor where care plan is unclearParticipating in team/group meetingsOther duties as requested or requiredQUALIFICATIONSPSW Certificate from an accredited training institution, required1–3 years PSW experience, preferredMember of the PSW Registry, preferred (may be required under some contracts)Current First Aid and/or CPR Certificate, requiredTraining in gentle persuasive/non‐violent crisis prevention, preferredTraining and previous work experience in Medication Reminders, preferredPrevious experience working in an Assisted Living environment, preferredAccess to reliable transportation, driver's license and vehicle insurance, requiredVulnerable sector police check at time of hire, annual declaration, required2‐step TB test at time of hire, requiredSt. Joseph's Home Care is an equal opportunity employer, committed to equity, inclusiveness, and diversity across all our programs, practices, facilities, and team.
