Key Responsibilities:
Support youth in improving mental and physical well-being
Facilitate referrals to both internal and external services




Collaborate with team members on personalized care plans
Assist youth in transitioning to adult services
Lead caregiver and youth peer support initiatives

Requirements:
Robust understanding of youth mental health care
Experience in community support and advocacy
Proven record of ethical practice and professionalism
Knowledge of the social determinants of health
