Wesley seeks a compassionate Health and Wellness Navigator to provide critical support for individuals in emergency housing. This full-time chance focuses on mental health assistance and harm reduction strategies.
In this role, you will work closely with participants to create individualized service plans while engaging with a community-oriented, multi-disciplinary team. Your expertise in crisis intervention and health support will be essential in empowering clients to achieve their goals. The position offers a chance to impact lives positively within Hamilton and Halton.
Key Responsibilities:
• Conduct thorough assessments and service planning • Assist with crisis resolution and participant safety plans • Foster relationships through regular participant engagement • Oversee harm reduction supplies and training programs • Collaborate with community partners on treatment options
Requirements: • At least three years in a related role • Degree in Human Services, Social Work, or similar • Preferred registration with OCSWSSW • Certification in Mental Health First Aid is advantageous • Must have a valid Driver's license and vehicle access
