Job ID # General Description The Stoney Health Services (SHS) Healthcare Aide (HCA) works collaboratively with the SHS multi-disciplinary health care team to provide basic health services and assistance/support with the activities of daily living for clients (and their families) who have chronic and/or complex medical conditions; physical, cognitive disabilities, and/or major functional limitations, as well as those clients requiring palliative and end-of-life care either in patient/client home settings in Mînî Thnî community. The range of care provided by HCAs is determined by individual client needs as well as the care setting and the competency of the individual HCA. Accountability HCAs are regulated health professionals in Alberta and are responsible and accountable for their own qualified conduct, ethical decision-making, competence, documentation, and practice within the HCA scope of practice. HCAs follow the client-specific care plan and work under the supervision, direction, assignment, and/or consultation of a licensed health care professional, such as the Home and Community Care Manager, RN, LPN, or other approved licensed supervisor/designate. HCAs must seek guidance, consultation, or direction when a client’s needs change, a task is unclear, the HCA has not been trained or deemed competent for the task, or the task may fall outside regulatory, employer, or individual scope of practice. Employer and individual scope of practice must not exceed the HCA regulatory scope of practice set by legislation, regulation, CLHA Standards of Practice, Code of Ethics, and related regulatory documents.



Duties and Responsibilities Person and Family-Centered Care Provide care that respects client dignity, autonomy, privacy, culture, language, personal values, and family/caregiver involvement, consistent with the client’s care plan. Assist and support activities of daily living, which may include bathing, grooming, dressing, toileting, other personal hygiene activities, as well as feeding, mobility, and exercise as needed. Assist with approved light housekeeping, meal preparation, and social contact supports when included in the care plan. Support client independence and self-care by encouraging participation in care to the greatest extent possible and recognizing the client’s strengths, preferences, and changing needs. Palliative, End-of-Life, and Comfort-Focused Support Provide palliative, end-of-life, and comfort-focused support within the HCA role, care plan, training, and direction of the RN, LPN, or supervisor/designate. Health Services, Observation, and Reporting Make and report client observations within the HCA role. HCAs do not diagnose or independently assess; concerns, changes, or risks must be reported to the nurse according to SHS process. Monitor and promptly report changes in physical condition, cognition, behavior, pain/comfort, mobility, skin integrity, intake/output concerns,



emotional or social concerns, safety risks, neglect or abuse concerns, and any unexpected client response. Support health routines, home exercise or strength-building programs, client wellness education, and chronic disease or palliative care supports only as authorized in the care plan and directed by the RN, LPN, or supervisor/designate. Assist with medication-related supports, treatments, equipment, basic foot care, or other health services only when the activity is within regulatory scope, allowed by SHS policy, included in the care plan, assigned by the supervising licensed health care professional, and supported by required training and competence. Restricted Activities and Supervision Requirements Perform restricted activities only when expressly authorized by applicable regulation, SHS policy, the client care plan, and assignment by the RN, LPN, or supervisor/designate. Complete client-specific education, training, and competency validation before performing any restricted activity or higher-risk task. Follow all additional instructions in the client care plan and document the care provided, client response, and any concerns according to SHS requirements. Do not proceed with a task if supervision, training, care plan direction, equipment, or safety conditions are not adequate; seek direction before continuing.
