Detailed Overview

Within the context of a client and family-centred model and in accordance with the vision and values of the organization, the Overdose Outreach Team (OOT) Outreach Worker works as a member of an interdisciplinary team to provide support services to individuals and their families with accessing opioid agonist therapy and related support services. The role involves monitoring clients by observing their health condition and environment, arranging and escorting designated or marginalized individuals to appointments and errands, creating linkages to community resources and programs, assisting clients with perks and assistance, providing brief planning and coordination on substance use, opioid agonist therapy, overdose prevention, harm reduction, and related social determinants of health. Under the direction of the Registered Nurse, the worker administers medication to clients and provides medication reminders, liaises with community resources and healthcare providers, and provides basic health-promoting information and life-skills development. The worker performs general clerical duties, attends meetings, and participates in patient care planning and coordination.

Responsibilities

1. Monitors clients by observing their health condition and environment, including functional ability, living arrangement, and family supports, documenting behavioural, physical and cognitive changes, and reporting significant and unsafe observations to the Registered Nurse or delegate.


2. Arranges, plans, escorts and accompanies designated/marginalized individuals to medical appointments and errands, providing emotional,
   
   
   
   moral support and physical assistance to further self-management skills.


3. Creates linkages to other community resources and programs, contacts relevant organizations or businesses, assists clients in processing forms, and provides advocacy as needed.


4. Assists clients in accessing benefits or assistance, and provides brief assessment and planning on substance use, opioid agonist therapy, overdose prevention, harm reduction and social determinants of health such as income security, housing, social networks, self-efficacy and self-care.


5. Under the direction of the Registered Nurse, administers medication to clients and provides medication reminders in accordance with established procedures, identifies potential emergency situations, and performs appropriate interventions such as CPR, contacting Mental Health Emergency Services, ambulance or police, and staying with clients.


6. Acts as a liaison between agencies, updating team staff on available services and liaising with community agencies for information on services.


7. Identifies client needs for services and makes appropriate referrals or develops alternate strategies for support, education and follow-up care.


8. Provides basic health-promotion information such as crisis counselling/prevention/intervention around issues such as violence, loss, grief and harm reduction.


9. Promotes development of life-skills, providing clients with information, referrals and practical assistance to meet basic needs and encouraging empowerment and capacity to exercise choice.


10. Performs clerical duties such as responding to calls, maintaining client files, using computer software and producing documents, files, photocopies and faxes.


11. Attends meetings and participates in patient care planning and coordination.


12. Maintains accurate records of outreach activities and provides regular reports.


13. Operates in accordance with health and safety requirements, reporting unsafe situations promptly.


14. Performs other related duties as assigned.

Qualifications

Education and Experience

Completion of Substance Use Certificate plus one year of recent related experience working with clients with substance use issues, or an equivalent combination of education, training and experience.

Valid BC drivers license and vehicle.

Current CPR certificate.

• Demonstrated ability to work with marginalized and culturally diverse populations including those with substance use issues and mental illness.


• Demonstrated listening and information-seeking skills that promote communication and lead to a cooperative approach to problem solving in a multidisciplinary setting.


• Demonstrated ability to establish rapport with clients and families to explore issues beyond the initial concerns.


• Working knowledge of harm reduction principles.


• Crisis prevention/intervention and conflict resolution/de-escalation skills.


• Ability to work independently and collaboratively in a team environment and partner with community agencies.


• Ability to communicate effectively both verbally and in writing.


• Physical ability to carry out the duties of the position.
