The Social Worker BSW will collaborate with Continuing Care and Home Care teams and families, clients/residents to establish a plan to meet the unmet needs for any issues and/or concerns of the resident/client/family in the areas of financial security and advanced care planning ethical decision making to identify,



assess and if possible resolve the concerns or problems for clients/residents of Continuing Care facilities, as expressed by the referral source by collaborating with the family, client and/or health care team. These concerns would focus on their financial needs and address social determinants of health.
