In this valuable role, the Clinical Supervisor is accountable for the supervision of community care assistants (CCA) and community care assistants in training (CCAIT) who are caring and making a difference for clients within the community. face to face meetings and one-on-ones.Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.What You BringRegistration as a Licensed Practical Nurse in good standingMinimum of 3‑5 years’ experience as a practicing nurseComprehensive patient health assessment skills are requiredPossess an understanding of the principles of community nursing practiceProven experience leading and managing a teamKnowledge of clinical documentation standards and practice guidelines is requiredExcellent organizational, interpersonal, and teaching skillsCommitment to professional development in clinical practice that is relevant to home health careEffective critical thinking and problem‑solving skillsMust possess a valid driver’s license and access to reliable transportationAble to provide a current, clear Vulnerable Sector Screening (VSS)Time TypeFull time#J-18808-Ljbffr
