Key Responsibilities:
• Collaborate on individualized service plans with clients • Assist clients in meeting CTO obligations and accessing services • Provide psychoeducation on illness management and medications • Foster respectful relationships with clients and support systems • Deliver advocacy to support client needs in the community
Requirements: • Degree or diploma in social services or related field • Three years' experience in community mental health • Strong knowledge of trauma-informed care practices • Ability to analyze and make decisions effectively • Valid Ontario Driver's License required
