Learn more.Queens Park Care Centre (QPCC) is located in New Westminster and provides rehabilitation services, an adult day program and a long-term care community. QPCC has an outpatient rehab service and 76 specialized inpatient rehabilitation beds. These provide intensive rehab for different patient populations including Brain Injury, Spinal Cord Injury and Stroke rehab patients as well as lower leg amputees, complex neurological conditions and other muscular skeletal injuries. Queens Park is a small and friendly site with a close link to its community and a high dependency on its multidisciplinary team. Come help us take Queens Park into the future -Apply today!Learn more about the dynamic city of Recent Westminster here: New Westminster.ResponsibilitiesProvides physiotherapy assessment of the patient's physical status, functional abilities, needs, and goals by methods such as reviewing records, gathering information, interviewing the patient, identifying patient and therapist expectations, selecting and applying evidence-based assessment methods, consulting with members of the health care team, performing examinations, and monitoring the patient's health to improve, maintain or support physical performance.Analyzes the patient's needs, establishes physiotherapy diagnosis and prognosis and develops an evidence-based, outcome-focused intervention strategy; utilizes assessment andreassessment findings, discusses the analysis with the patient and caregivers, develops short and long term goals and selects a practice model/intervention consistent with patient needs and goals to achieve optimal health outcomes.Implements interventions in accordance with established treatment guidelines and educates patients and caregivers regarding self-management, health promotion and injury/disability prevention.Participates in the interprofessional care planning process for patients as appropriate by ensuring the plan is established and implemented effectively, updating care plan and intervening as appropriate, and collaborating and communicating on issues that will affect the care/
