The ideal candidate will have experience in medical or dental office settings, familiarity with electronic medical records, and robust communication skills to provide exceptional service in a fast-paced environment.
Duties
Conduct intake consultations and assessments
Develop and update care plans
Coordinate staff scheduling and coverage
Communicate with families and referral partners
Maintain accurate documentation and records
Qualifications
Background in health administration, social work, or care coordination
Strong organizational and communication skills
Experience in home care or community services (asset)
Comfort with scheduling software and documentation
