Key Responsibilities:
• Coordinate health and social service needs for marginalized populations
• Provide consultation and assessment for community health services
• Implement service plans in collaboration with agency staff
• Adhere to privacy and documentation standards
• Increase individual and organizational capacity in service delivery
Requirements:
• Four years of community experience, two in clinical coordination
• Relevant health or human services degree required
• Non-violent Crisis Intervention Training
• Valid driver’s license and vehicle required
• Member in valuable standing with relevant professional organization
