Join Threshold Housing Society as a Connections Case Manager, where you'll provide critical support to homeless youth. Engage in meaningful relationships and flexible outreach activities.
In this temporary full-time role, you'll focus on trauma-informed assistance for young people aged 16-25 facing housing instability. Your role will include system navigation, advocacy, and support to promote long-term success for each youth. Collaborating with community organizations, you will deliver hands-on assistance while adapting to various schedules, including evenings and weekends for program delivery.
Key Responsibilities:
• Implement early intervention strategies for at-risk youth
• Support youth in establishing personal goals
• Facilitate community-based outreach initiatives
• Advocate for comprehensive and culturally safe resources
• Coordinate with organizations to enhance youth stability
Requirements:
• Degree in Social Work or relevant field
• Experience working with vulnerable youth (2-3 years)
• Robust skills in case management
• Familiarity with social media and technology
• Emergency First Aid certificate
