As a Registered Practical Nurse, your shifts will be from 8am-8pm, with rotating weekends. This full-time position prioritizes patient relationships, relying on your compassionate care and critical assessment skills. You will play an integral role in delivering safe and ethical nursing, engaging in meaningful interactions with patients in the community.

Key Responsibilities:
• Conduct scheduled visits to assess patient health
• Provide protected, ethical nursing care daily
• Utilize assessment tools to identify patient needs




• Execute and adapt individualized care plans
• Foster trust with patients to enhance care delivery

Requirements:
• CNO registration as a Registered Practical Nurse
• Driver's license and access to a vehicle essential
• Experience in community care is preferred
• Demonstrated reliability and flexibility for patient travel
• Capable of on-call responsibilities outside regular hours
