This role at AgeCare is crucial for overseeing the administration and implementation of the MDS process. As the RAI Coordinator, you will provide essential training and support to ensure the interdisciplinary team adheres to documentation requirements. Key Responsibilities:
• Administer and coordinate the Minimum Data Set (MDS) process
• Train interdisciplinary staff on MDS documentation
• Monitor and audit completion of MDS assessments




• Develop and manage the resident assessment schedule
• Assist in coding assessments for accuracy and compliance

Requirements:
• Registered Nurse (RN) or Registered Practical Nurse (RPN) qualification
• At least three years in Long-Term Care settings
• Familiarity with Long-Term Care Act and Ministry standards
• Proven project leadership and organizational abilities
• Strong focus on resident care and teamwork
