Become a vital part of the Population Health Clinic as a Registered Nurse. Help bridge the gap in health care access for residents in East Toronto through effective collaborative practice.
This full time role emphasizes client-centered care and teamwork within a growing interdisciplinary primary care setting. You will work closely with health professionals to deliver targeted education, conduct assessments, and provide support for vulnerable populations. Your expertise will contribute to health promotion and community projects.
Key Responsibilities:
• Coordinate internal and external health care services
• Deliver tailored health education and support programs
• Conduct follow-ups and triage patient needs
• Facilitate home visits for home-bound clients
• Engage in quality improvement initiatives and committees
Requirements:
• RN registered with the College of Nurses of Ontario
• Bachelor’s degree in nursing required
• At least three years of relevant nursing experience
• Proficient in patient assessment and counseling
• Knowledge of chronic disease management practices
