You will work closely with health professionals to deliver targeted education, conduct assessments, and provide support for vulnerable populations. Key Responsibilities:
• Coordinate internal and external health care services
• Deliver tailored health education and support programs
• Conduct follow-ups and triage patient needs




• Facilitate home visits for home-bound clients
• Engage in quality improvement initiatives and committees

Requirements:
• RN registered with the College of Nurses of Ontario
• Bachelor’s degree in nursing required
• At least three years of relevant nursing experience
• Proficient in patient assessment and counseling
• Knowledge of chronic disease management practices
