Transform community health as a Nurse Practitioner with Kingston Community Health Centres in Napanee. Your focus on holistic care and equity will enhance health outcomes for diverse populations.
In this permanent, full time position, you will deliver primary care in collaboration with community Physicians, providing a broad range of services tailored to clients’ cultural needs. You will participate in mental and physical health programs while addressing social determinants of health to inform your practice and client interactions.
Key Responsibilities:
• Conduct health assessments and prescribe necessary treatments
• Lead specialty clinics in collaboration with healthcare teams
• Maintain client records and participate in quality improvement initiatives
• Advocate for health resources and client support
• Educate clients on health and lifestyle changes
Requirements:
• Registration as a Nurse Practitioner in Ontario required
• Minimum of 3-5 years nursing experience in primary care
• Experience with Indigenous and marginalized communities
• Familiarity with trauma-informed care practices
• Reliable access to transportation for community outreach
