Conduct scheduled visits to care for patients in various settings
Care for patients of all ages with differing complexity of care needs
Help patients remain comfortable in their homes for as long as possible
Use appropriate assessment tools and techniques to assess patients’ individualized needs
Execute and continuously monitor care plans
Engage patients using a team-oriented approach by building rapport and trust
Maintain and update professional patient records related to patient encounters and plans
Provide required on-call coverage, including after hours
Requirements
Registration with the CNO as a Registered Nurse in good standing
Valid driver’s license and access to a vehicle is required
Experience in delivering community care is an asset
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Required on-call coverage,
including after hours to ensure patient safety and to meet patient care needs

Demonstrates expertise in patient care management, utilizing assessment tools to tailor care plans and ensuring patient comfort and safety. Strong ability to engage with patients collaboratively while maintaining accurate records and providing on-call support.
