In this role, you will assist individuals to engage in community opportunities tailored to their unique preferences. Your focus is on empowering clients to advocate for their own needs while ensuring their rights are respected. With at least two years of experience and relevant training, you bring a team-oriented spirit and a commitment to community integration.
Key Responsibilities:
• Advocate for the rights and preferences of individuals supported
• Facilitate community participation in social and vocational activities
• Foster relationships with families and professionals
• Implement and document the Individual Service Plan
• Maintain daily records of activities and health information
Requirements:
• Grade 12 education and relevant diploma
• Minimum 2 years in community services or healthcare
• Current First Aid/CPR and Food Safe certifications
• Valid BC Driver’s License and clean driving record
• Criminal record clearance for vulnerable populations
