In this position, you will report directly to the Director of Care, serving as a vital member of the interdisciplinary healthcare team. Your responsibilities will include conducting comprehensive client assessments and developing personalized care plans to address physical and psychosocial needs. You'll monitor ongoing client conditions and ensure effective care delivery while participating in team collaboration.

Key Responsibilities:
• Complete comprehensive client assessments
• Develop and implement personalized care plans




• Administer prescribed medications accurately
• Monitor ongoing client conditions regularly
• Collaborate with the interdisciplinary team effectively

Requirements:
• Graduation from a recognized LPN program
• Current active registration with CLHA
• Current Basic Life Support CPR certification
• Nursing experience preferred
• Demonstrated leadership skills required
