CenterWell Home Health is seeking a dedicated Home Health Physical Therapy Assistant to join our patient-centered care team. You will work onsite in patients' homes, implementing the physical therapy plan under the supervision of a Physical Therapist, to help improve strength, mobility and independence. Responsibilities include administering active and passive therapy, documenting records, coordinating with the clinical team, and ensuring infection control and safety protocols are followed.
