Join our Integrated Care Team and make a meaningful difference in the health and well-being of individuals, families, and communities. As a Community Health Nurse, Primary Care, you will play a key role in delivering accessible, patient-centered healthcare services while promoting wellness, disease prevention, and healthy communities.

This is an exciting prospect for a collaborative and community-focused nurse who is passionate about improving health outcomes and helping build healthier communities.

Key Responsibilities:

- Deliver primary care and community health nursing services, including health promotion, disease prevention, immunizations, and chronic disease management.
- Assess, plan, implement, and evaluate patient care to support positive health outcomes across the lifespan.




- Coordinate care for patients with complex health needs through case management, referrals, follow-up, and collaboration with interdisciplinary team members.
- Monitor and support preventive health screening and ongoing management of acute and chronic conditions.
- Educate and empower patients and families to make informed decisions about their health and wellness.
- Provide culturally safe, trauma-informed, patient-centered care while advocating for equitable access to health services.
- Maintain accurate clinical documentation and contribute to quality improvement and community health initiatives.
- Collaborate with healthcare providers and community partners to promote coordinated, high-quality care.

Key Qualifications:

- A Nursing Degree, AND
- Two (2) years of recent nursing experience in an ER setting or within a Community Health Centre or Public Health Unit.

Additional Requirements:

- Current registration with the College of Nurses of the Northwest Territories and Nunavut must be obtained and held thru employment.
