Within the Primary Care team, you'll drive the coordination of care by crafting client-centered care plans. The Client Navigator collaborates with clients and service partners and advocates for individuals facing health inequities, promoting strength-based solutions to their needs.
Key Responsibilities:
• Help clients achieve their identified health goals
• Facilitate both internal and external referrals
• Support clients with documentation for disability claims
• Work alongside legal services for client appeals
• Advocate for health services and community programs
Requirements:
• Bachelor’s degree in social work or related field
• Experience in community healthcare settings
• Strong communication and advocacy skills
• Familiarity with Ontario's health service landscape
• Flexibility to work during evenings and weekends
