Join Covenant House as a Follow Up Worker, supporting young people in Toronto achieve housing stability. This full-time role involves crucial support in navigating community resources and services.
As a Full time Follow Up Worker, you will assist clients with housing assessments and develop personalized plans to support their transition out of homelessness. Additionally, you’ll provide life skills training and advocate for clients within various support systems to ensure comprehensive assistance and long-term success.
Key Responsibilities:
• Conduct initial assessments with clients for housing options • Develop individualized housing and service plans • Provide life skills training tailored to each youth's needs • Liaise with housing providers and coordinate care • Maintain documentation on client progress
Requirements: • College or university education in youth work or related • Experience with housing systems and client navigation • Strong communication and interpersonal skills • Knowledge of youth homelessness barriers • Commitment to inclusive and anti-oppressive practices
