Lead initiatives in person-centered support for individuals with disabilities at Winnifred Stewart. This role emphasizes assessment, planning, and community involvement in a supportive workplace.
As a Service Coordinator, you'll leverage extensive experience in disability services to support individuals in developing personalized plans that enhance community participation and personal care. Collaborate with clients to advocate for their needs while connecting them with recreational and social opportunities. You will work as part of a dedicated team focused on improving the lives of those you support.
Key Responsibilities:
• Develop and document individualized support plans • Facilitate communication within support groups • Advocate for individual rights and life choices • Cultivate recreational and social opportunities • Provide coaching for community participation
Requirements: • Valid Class 5 Driver’s License and insurance • Ability to work effectively within a team • Basic computer and writing skills • Strong observation and communication skills • Knowledge of community resources and support networks
