Key Responsibilities:
• Perform detailed assessments including ADL and IADL evaluations
• Design evidence-based plans targeting safety and functionality




• Manage follow-up visits to assess client progress
• Recommend and order assistive devices as needed
• Collaborate on care transitions with other healthcare professionals

Requirements:
• Registered member of COTO in good standing
• Experience in home care and community support preferred
• Strong documentation and organizational skills
• Ability to work independently and efficiently
• Access to reliable transportation a must
